MNDSA helps Minnesotans with disabilities and their families navigate Medicaid waivers, build person-centered support plans, and connect to the community resources they need — on their own terms. Our staff share lived experience with disability. We work across all 87 Minnesota counties.

For Individuals & Families

Navigating waivers, CDCS, and HCBS services can feel overwhelming. We make it simple — and we've been there ourselves. Our team includes people with disabilities and family caregivers who understand what you're going through.

For Providers & Professionals

Refer a client, request HCBS training for your clinic or hospital, or become a B.R.I.D.G.E. mentor. We work with therapy centers, county offices, hospitals, and social workers statewide.

Services Overview

Support Planning

Person-centered CDCS support plans for Medicaid Waiver recipients in all Minnesota counties. Billed through your Fiscal Management Service. We help you write your plan, navigate your options, and solve problems along the way.

B.R.I.D.G.E. Program

The B.R.I.D.G.E. Program connects individuals with disabilities and their caregivers to personalized resources through peer mentorship and advocacy. Funded by the DHS HCBS Connectors grant.

Multilingual county services guides, HCBS waiver overviews, disability resource links, and home education support — all in one place. Guides available in English, Oromo, Somali, and Spanish.
